APPLICATION FOR DOG TRAINING CLASS

Thee first day of the Beginner obedience/agility or Puppy Headstart class is orientation as well as some basic dog training commands. Please be sure to bring your dog to each night of class, including the first night. It is very important that you can attend each class as new skills will be taught each week.  If you enroll in the Beginner Home Manners or Puppy Headstart class, you will receive a book and training manual. You will need:

ENROLLMENT

1. To enroll, fill out and detach form below, indicating chosen class and time.
2. Fill out and attach enclosed Management Questionnaire Sheet.
3. Include appropriate class fee (see class schedule for amount).
4. Also include a photo copy of your dog's shot records we can keep for our files. NOTE: Rabies shot and Bordetella (kennel cough) are mandatory and  MUST be given by a Veterinarian. Please be sure your dog is free of parasites also.

GENERAL INFORMATION

Proper training equipment is required. This will be covered during orientation. Collars and leashes will be available for sale. Please do not buy equipment until after orientation.  We will help you fit a collar based on your dog's size and temperament.

Aggressive dogs are not allowed to work in group classes. "Aggressive" means the dog threatens to either bite or fight. If you feel your dog may be aggressive towards people or other dogs, please call us at 417-623-5606 or email us at positivedogs@joplindogs.com and someone will contact you.

Classes are limited to 10 students. Each class meets once a week for approximately 45 minutes to one hour for 6 weeks unless schedule indicates otherwise.  Classes fill quickly so be sure to get your application in as soon as possible. Your application will be confirmed by mail or phone.

Classes are held at 4191 Cactus Lane, Joplin, MO. Take 7th Street, West to Black Cat Road, turn North on Black Cat Road and go about .8 mile, turn right on Cactus Lane, proceed about 1/2 block. The building is on the left. The building is a big metal building with a Tri-State Kennel Club sign above door.
